Resumen
Heart failure is a chronic, progressive disease that is one of the leading causes of morbidity and mortality worldwide. Its management is based not only on in-patient and pharmacological interventions, but also on appropriate patient education and structured continuity of care strategies. Based on this problem, an institutional analysis was conducted at the Hernando Moncaleano Perdomo University Hospital in Neiva. Based on data presented at the April nursing selfmanagement meeting, it was found that of the 23 patients discharged with a diagnosis of heart failure, 3 were readmitted to the hospital for the same condition, representing 13.04% of discharges. This figure demonstrates a negative trend that can affect the patient's clinical stability, the functioning of the healthcare system, and the quality of the hospital discharge process.
