Hipotiroidismo primario asociado a tiroiditis crónica con tiroideomegalia bilateral en una canina geriátrica: Reporte de caso clínico en la clínica veterinaria de pequeños animales de la Universidad Antonio Nariño

Resumen

Bilateral thyroid enlargement in geriatric canine patients represents an uncommon clinical finding that requires a comprehensive clinical approach, as it may be associated with both benign and malignant processes. This report describes the case of a 13-year-old spayed female Pinscher dog that was evaluated at the Veterinary Clinic of Antonio Nariño University. The patient presented with progressive weight loss, anorexia, vomiting, hemorrhagic diarrhea, and palpable bilateral thyroid enlargement. Complementary diagnostic tests revealed decreased total thyroxine (T4), thyroidstimulating hormone (TSH) within the reference range, neutrophilic leukocytosis, increased gamma-glutamyl transferase (GGT) and creatine phosphokinase (CPK) activities, and cytological findings consistent with reactive neutrophilic thyroiditis, without cytological criteria of malignancy. Diagnostic imaging confirmed bilateral enlargement of the thyroid tissue with a compressive effect on the cervical region. A presumptive diagnosis of developing hypothyroidism was established. Treatment with sodium levothyroxine, hepatoprotective agents, gastroprotective therapy, and nutritional support was initiated. After 25 days, the patient showed clinical improvement, increased appetite, and radiographic reduction in thyroid gland size, highlighting the importance of a multimodal diagnostic approach in geriatric canine patients with inconclusive thyroid profiles.
